Symptoms: Depression, Egotism, Suicidal tendencies

Brief description of patient: Patient seems to have an unhealthy fascination with suicide, prone to emotional outbursts (bipolar disorder?). Thinks highly of himself, obsessive about his emotions, particularly love. Out of touch with reality, feels more at home with children than adults. Condescending to the poor and unfortunate. Displays little or no motivation for anything. Estranged mother, patient was possibly neglected during childhood?

Emotional state: Overall unstable, keep watch.

Suggested treatment: Bring in mother, hold conferences (separately and together). Assign work (gardening?) in new town. Frequent follow-ups, nurse visits daily, assign exercise-- no correspondence with "Lotte," allow visits from "Wilhelm".
